LEAITP Request Form (PDF)

Use this document to request an Export LEAITP. It can either be faxed or emailed to TMB. Instructions are on the form. The requesting agency/organization is responsible for securing the training location (two classrooms required for parts of the program) and for arranging for area lodging for instructors and participants. Instructor travel and per diem costs are billed to the requesting agency. The requesting agency will be required to reimburse FLETC for training materials.
